- Routine Dental Exams (2 per year)
- Bitewing + PA X-Rays (as needed)

- Fluoride (2 per year for 12 yrs old or younger)
- $300 Off Teeth Whitening (50% off regular price)
- $500 Off Orthodontic treatments
- 15% Off Additional Preventative Services (sealants)
- 15% Off Basic Services (fillings, extractions) & Scaling & Root Planing
- 20% Off Major Services (crowns, bridge, denture, partial, onlay/inlay), Endodontics
PERIODONTAL PLAN INCLUDES: All of the same offered services, discounts and specials as the PPDA Adult Plan with the exception of/and including:
- Periodontal Maintenance Cleaning (4 per year)
SENIOR PLAN INCLUDES: All of the same offered services, discounts and specials as the PPDA Adult Plan with the exception of/and including:
- Removable Appliance Check-up, Cleaning, and Adjustments (2 per year)

- Payment due in full at time of sign up
- Effective the day of sign up
- Fees and services guaranteed for 1 year to date
- Plan fees are NON-REFUNDABLE for any reason
- Qualified child is 12 years old or younger
- Services are for covered member only and cannot be transferred to another β’ This is NOT dental insurance and cannot be combined with insurance coverage
- Services cannot be rolled over into a new year.
- $80.00 FAIL FEE FOR ALL NO SHOW, CANCELED, CHANGED OR FAILED APPOINTMENTS WITHOUT A MINIMUM OF 48 HOURS NOTICE. NO EXCEPTIONS!
